State Medicaid exclusion lists, state by state

Where each state publishes its Medicaid exclusion, sanction or termination list, what format it comes in, and which pages we could confirm. Checked October 4, 2026.

Not legal advice. States move these lists often. This page records what we found on October 4, 2026. Confirm with the state Medicaid agency before relying on any list, and check each list’s own “as of” date.

The short version

  • Federal lists come first. Check the HHS-OIG List of Excluded Individuals/Entities (LEIE) and the exclusion records in SAM.gov for every provider, every month.
  • Then the states where the provider is enrolled or practises. Each state Medicaid program keeps its own list of excluded, suspended or terminated providers. Names, formats and locations vary widely.
  • What we found: 34 state pages confirmed on October 4, 2026; 10 pages known but not confirmable by automated check; 7 states where we could not locate a current public list.
  • Formats vary. Some states publish Excel or CSV files, others a PDF, a search page or a table on a web page. A few files change their name every month.

Federal sources to check for every provider

State Medicaid exclusion lists

“Page confirmed” means the official page resolved and, for downloadable lists, the file was fetched and its format checked on October 4, 2026. “Not confirmed” means a page is known but could not be checked automatically, usually because the site blocks automated requests. “Not located” means the previously published location no longer works and we did not find a current public list.

How to screen against a state list

  1. Work out which states apply. At minimum, the states where the provider holds a license, is enrolled in Medicaid, or treats patients. Your provider agreements and payer contracts may add more.
  2. Get the current version. Download the latest file or run the search, and note the date the state shows on it. Keep a copy or a screenshot with the date for your records.
  3. Match on more than the name. Use NPI and license number where the list has them; fall back to name plus city, date of birth or provider type.
  4. Adjudicate possible matches. Record who reviewed each one, what identifiers they compared and the outcome: cleared, or confirmed.
  5. Repeat on a schedule and whenever you add a provider. Monthly is the common rhythm because the OIG LEIE is updated monthly.

Our exclusion screening guide walks through the full process, including what to do with a confirmed match.

How we compiled it

We started from the program integrity pages of each state Medicaid agency and followed them to the published list. For every list we could reach, we opened the page, downloaded the file where one exists and checked its format and columns. Pages that blocked automated checks or no longer resolved are marked as such rather than guessed. The same registry drives state list screening in Credentik. If you know where a missing list now lives, email hello@credentik.com and we will check and add it.

Questions

Frequently asked

Do I need to check state Medicaid exclusion lists as well as the OIG list?
Often, yes. The OIG LEIE covers federal exclusions only. A provider can be terminated or excluded by a state Medicaid program without appearing on the LEIE, and state programs and health plans commonly expect screening against the lists for the states where a provider is enrolled or practises. Check your state Medicaid provider agreement and any payer contracts for the exact requirement.
How often should we screen?
The HHS Office of Inspector General updates the LEIE monthly and says monthly screening of employees and contractors best minimizes potential overpayment and civil monetary penalty liability. Federal rules require state Medicaid agencies to check the LEIE and the federal exclusion list in SAM no less frequently than monthly (42 CFR 455.436). Many organizations adopt the same monthly rhythm for state lists, but publication schedules vary, so check the date on each state's file.
What does “not confirmed” mean in the table?
We found a page for the list but could not confirm it on October 4, 2026, usually because the site blocks automated checks or renders the list only in a browser. Open the link yourself, or contact the state Medicaid agency's program integrity unit.
What should we do when a name matches?
Treat a name match as a possible match, not a finding. Compare identifiers such as NPI, license number, date of birth and address against the list entry. Document how you confirmed or cleared it. If it is a true match, follow your compliance policy and your contracts, which usually means stopping the provider from billing the affected program and talking to counsel about any claims already paid.
Does Credentik screen against these lists?
Credentik screens providers against the OIG LEIE, SAM.gov and the Medicare opt-out list, plus 19 state Medicaid exclusion lists checked automatically (the states that publish a downloadable file, imported on a schedule); lists that are published only as a PDF or a search page can be loaded from an export. Possible matches go to your team to confirm or clear, and a monthly attestation report summarizes the checks.
